Group Term Life Insurance
This is an Employer Funded plan, with four plan options to choose from to fit your agency’s needs. The benefit is guaranteed issue and offers the option to add spouse and dependent children up to age 19, or to age 25 if a full time student. (Certain Requirements must be met).

Voluntary Life Insurance
The beauty of this benefit is that it can be offered as an Employer Funded plan or offered on a Voluntary basis, at the employees expense. Coverage is Guaranteed Issue up to $100K on employees; up to $30,000 for spouses and up to $10,000 on dependent children to age 19, or to age 25 if a full time student, provided certain requirements have been met. Benefit amounts of up to $500,000 or 5 times salary are available to employees with Evidence of Insurability.

Group Short Term Disability
The Short Term Disability program protects for loss of earnings during an illness and/or accident, to include Maternity. The benefit pays 60% of earnings to a maximum weekly benefit of $1000 for a period of 12 weeks, once the 7 day elimination period has been met. This benefit can be offered on an employer paid basis or on a Voluntary basis at the employees expense. Coverage is Guarantee Issued, provided certain requirements have been met.

Group Long Term Disability
Protect your employee’s financial security by offering Long Term Disability coverage. The benefit pays 60% of earnings to a maximum monthly benefit of $10,000 after the 90 day elimination period has been met. The benefit includes disability plus, a minimum benefit and a Survivor Benefit. Coverage is Guarantee Issued, provided certain requirements have been met.

Group Dental & Vision Insurance
Underwritten by Guardian Life Insurance Company, this program gives you the option to enroll in one of two dental plans offered, as well as a separate Vision plan. Plans can be offered as an employer paid benefit or offered on a voluntary basis. Our Guardian dental plans give you the choice of going to an in or out-of-network provider, with the Vision plan using the VSP Provider network. The vision plan provides a $150 allowance on frames and contacts and co-payments for your annual eye exam and materials.
